I like the online Vial of Life form better than the old hand written ones, but if you want the green alert sticker for your windows you have to phone in for those.
The Vial of Life is a form for personal and medical info that should be completed for each family member. The completed form(s) are to be placed in the plastic pouch provided and attached to the refrigerator so it is accessible to emergency responders. The vial of life packets have been available at the UCO office.Not sure about now, but they were there when I worked there
